I am going on a (plane) trip with type 1 diabetes



 

I’m going on a trip with type 1 diabetes and I’m bringing… a travel declaration. This document is useful if customs or security ask difficult questions. So make sure to keep it in your hand luggage.

We only issue a travel declaration if you are being treated by us. If you have a different diabetes care provider, please contact your own diabetes team.

CONTENTS OF THE TRAVEL DECLARATION


In the English-language travel declaration, we—as your medical provider—confirm that you must be allowed to carry insulin in your hand luggage and have access to it at all times. We also state that you must always be able to check your blood sugar. Our travel declaration is valid for six months from the date of issue.

Additionally, we indicate whether you use an insulin pump (and which type), whether you inject insulin, and the name of the insulin you use. We also mention whether you use a FreeStyle Libre or a continuous glucose sensor. Very useful at customs!

REQUESTING YOUR TRAVEL DECLARATION ON TIME


Request your travel declaration in advance using the form below. Do this at least five working days before your departure. We will send the declaration to you by email with a valid digital signature from the doctor.

Prefer to carry a paper version? Print the declaration or ask for a printed copy at the reception desk during your last clinic visit before your holiday.

Note: Airline requirements for the issue date of a medical declaration may vary. Be sure to check with your airline for their specific requirements.
