Sharing glucose data with your diabetes care team


You can share data from your glucose sensor, insulin pump, or AID system with your diabetes care team. This allows our healthcare professionals to provide more accurate and personalized advice for your treatment.

Insights into glucose data

 

Glucose sensors and insulin pumps collect a lot of information. You can often easily view this data yourself via a software program provided by the manufacturer.

If the correct data is linked to your file at Diabeter, your diabetes care team can access it when needed. With this information, our healthcare providers better understand your situation. Your diabetes team can also give better advice about your treatment.

The data you share with Diabeter also appears automatically in the Diabeter App. Your glucose values are displayed in a heatmap. This is an overview where your blood sugar levels from the past two weeks are color-coded. Low glucose values are blue. Blood sugars within the target range are green. High glucose values are yellow, orange, or red.

How you can share data with Diabeter depends on the system. Below, we provide an explanation for each software program.

Carelink

Medtronic’s systems work with the software program Carelink. With your own account, you can view and share your data with your treatment team.

To do this, provide your username and password to your diabetes nurse. They will ensure that the data from Carelink is added to your file at Diabeter.

Glooko

This software program is used with various pumps and glucose sensors, including Omnipod, Dexcom, Tandem, and Ypsopump. On Glooko’s website, you’ll find a list of all compatible systems, along with an explanation of how the software works.

There are two ways to share data from Glooko with your treatment team:

  • Via the Glooko website: Log in to your Glooko account. Instructions on how this works can be found [here].
  • Via the Glooko app: You’ll need a ProConnect code from Diabeter. Your treatment team can provide this to you.

Libre View

Abbott offers Libre View as a software program for sharing data. It works with the FreeStyle Libre. With this program, you can view and share glucose data online with your care team.

THERE ARE TWO WAYS TO SHARE DATA FROM LIBRE VIEW WITH YOUR CARE TEAM:

  • Manually, via your Libre View account. Under ‘Share Data’, you enter the practice ID of Diabeter. Your care team can provide this to you.
  • Your care team invites you.You’ll receive a link from Diabeter’s account in Libre View. This will be an email explaining exactly what you need to do.

 

 

Why can’t I share my data?

Are you using other software to share your data? That’s certainly possible. For example, Clarity, Yourloops, or programs for open-source systems. These cannot be directly linked to your file at Diabeter. However, there are other ways to share your data.

 

Clarity


This is a program from Dexcom. You can link Clarity to Glooko. This way, you can still share your data with your care team.

Yourloops


For this program, your care team can send you an invitation to connect. Your healthcare providers will then be able to view your data. Unfortunately, this data is not visible in your file at Diabeter or in the Diabeter App.

CamAPS


With this program, it’s also possible to connect with Glooko.

OTHER SYSTEM?


For some other systems, solutions are also available. You can ask your care team about this. Your diabetes nurse knows what’s possible and is there to help you further.
