Sometimes important details change—for example, if you switch general practitioners or pharmacies, move to a new address, or get a new email address. You can easily report these changes using this form.
Which information is important?
If you are receiving treatment at Diabeter, it’s important that the information in your file is accurate. This includes three types of data:
- Contact details: A correct email address and phone number ensure we can always reach you when needed.
- Personal information
- Healthcare providers and insurer: Accurate details about your general practitioner, pharmacy, or other healthcare providers help us share information with them when necessary. We also need to know which health insurance company you’re insured with.
Submit changes
Has something changed in your situation? Let us know online right away. We’ll update your file at Diabeter accordingly.
